Experimental immunotherapy treatment used to fight breast cancer

The type of hormone therapy administered was dependent upon whether women were premenopausal or postmenopausal-91% of postmenopausal women received an aromatase inhibitor-based regimen, whereas premenopausal women were most commonly prescribed either an endocrine therapy regimen comprising either tamoxifen alone or tamoxifen followed by an aromatase inhibitor (78%). Treatment options include estrogen-blocking drugs along with doses of chemotherapy to lower the risk of recurrence. In addition, these early tumors must be sized at five centimeters or less and not invasive to the lymph nodes.

The Oncotype DX genetic test has been available on the NHS since 2013, but the National Institute for Health and Care Excellence (Nice) is now updating its guidance on whether it should be recommended for use. But the researchers who conducted this new controlled experiment found that chemotherapy provided no additional benefit over hormone treatments alone. The test randomized women with intermediate risk, about 67 percent. They benefit just as much from chemotherapy, which many don't tolerate well and can have long-term consequences, as they do from hormone treatments, which have many fewer side effects.

While IM-MS nodes can be treated with radiation therapy, concerns about the side effects of increasing the radiation area to the chest has limited such use.

To make this therapy the team first tested the tumour genetically and looked at the different properties of the cancer that would make it a target of the immune system. For the group that had both hormone and chemotherapy, the rate was 84.3%, a difference that was not statistically significant.

Researchers at the National Cancer Institute (NCI) examined over 10,000 breast cancer patients and found that women who were treated with an estrogen-blocking hormone therapy had a almost identical survival rate to women treated with chemo and hormone therapy. Because of that, many women with early-stage cancer used to be urged to get chemotherapy in hopes of preventing any spread.

But there is a note of caution in interpreting the study's findings.

About 17 percent of the women tested had high-risk scores and were advised to have chemotherapy.

The result: After an average follow-up of 7.5 years, the researchers saw no added benefit in this group to adding chemotherapy to the treatment mix.
